If you are a physician currently working with RPAP students in your community and desire to have an adjunct faculty appointment in the Department of Family Medicine and Community Health, click on the form links below. Print all the pages, fill them out, and return to the RPAP Office. Your application will be forwarded to the Department of Family Medicine and Community Health at the Unversity of Minnesota Medical School.
