Instructions to Donors

Under the current statutes of the State of Minnesota, it is possible for an individual to bequeath his or her body to the University of Minnesota.

STEPS TO TAKE TO BEQUEATH YOUR BODY TO THE UNIVERSITY OF MINNESOTA MEDICAL SCHOOL:

Step 1
Call or e-mail The Anatomy Bequest Program at 612-625-1111 or bequest@umn.edu to request that a bequest form be mailed to you.

Step 2
The bequest form should be completed, signed, witnessed by two people, and the original form returned to the following address:

If this bequest is made following an advance health care directive, please enclose a copy.  The original bequest will be filed at the University and three copies of your bequest will be returned to you.

Step 3
We suggest that the bequest forms be distributed to those persons who would be notified at the time of your death. It is important that your physician, your family and/or friends know of your bequest, so they can make sure your wishes are carried out.
