Primary Care Vision Statement

The Primary Care Center will provide patient-centered care using evidence based medicine and the chronic care model through a team-based approach.  This will assure patients ready access to their care team.  We will be a leader in the search for innovations in care for our patients and in teaching the next generation of health care providers.  In so doing, we will become a workplace in which our providers and staff will thrive.
