Administrative Changes in Program Form

This form provides programs with a standardized tool to document and communicate, internally, administrative changes in their program.  It is critical that administrative changes are reported to the assigned GME program specialist, prior to their effective date, so that all internal GME documentation associated with the change, i.e. listserves, website, sign-in sheets, etc. are updated in a timely manner.

Submission of this form for internal action, does not eliminate the requirement that programs complete and process documentation required by external agencies, such as the ACGME, to initiate changes with them.

Administrative Changes in Program Form (Word fillable form)

Steps to submit form for internal action:

1. Program downloads the form, and completes the appropriate fields to document the change
2. Program submits the completed form as a PDF via e-mail to the assigned GME program accreditation specialist.
3. GME program accreditation specialist processes all changes internally.
4. GME program accreditation specialist notifies the program when all internal changes have been made, and files the form in GME.
