Immunization Compliance Requirements and OHS Services
New: 2011 Flu Shot Campaign Seasonal flu shots are important, and many of our affiliated hospitals require all trainees to participate in the Flu Vaccination Program. Please remember to get your flu shot! Introduction: All residents and fellows must meet University of Minnesota Academic Health Center immunization compliance requirements throughout their residency/fellowship. Your immunization record is maintained by Boynton Health Services (BHS). Please note: If you receive your immunizations at BHS, they will charge you. Please submit your completed immunization record and all subsequent immunization information directly to BHS at: Boynton Health Services 410 Church St SE Minneapolis, MN 55455 Fax: 612-626-9768 Questions about your immunization status: Contact BHS via email or call them at 612-626-5571. Trainees may decline the hepatitis B vaccine. If you decline, you must complete the Hepatitis B Vaccination Declination form and submit it directly to BHS as listed above. If you have not completed all immunization requirements prior to your start date, you must attain compliance as soon as possible with your primary care physician or clinic and provide the updated immunization record to BHS. The University does not cover the cost of immunizations. Throughout your training you will be required to stay compliant with immunization requirements. The Occupational Health and Safety (OHS) offices at many of our affiliated sites are open for immunization services for residents and fellows at no cost. These services include annual Mantoux updates (required when you start and in September of each subsequent year), hepatitis B vaccine, chicken pox and other immunizations. Anyone who has tested positive is required to complete the Annual Symptom Survey and submit it to BHS. (Note: when accessing the Annual Symptom Survey scroll down mid-page. This document is designed as a shingle to go in the medical record). Note: The BHS Clinic held each month in the CHIPS lounge is for medical students only. Residents and fellows cannot use this clinic. Immunization Status** | Click on the link above to access your University Portal (myUportal). Log in using your x.500 and password. You can find immunization information under "My Health and Wellness" tab. Immunizations are in a box with a red cross icon. When you receive an e-mail notice that you are non-compliant: - present a hard copy of the e-mail to the OHS staff at the affiliated hospital site and
- request walk-in immunization service or make an appointment
If you are unable to print the email notice or more information is required you will be permitted to use a computer at the OHS offices to access your University portal (using your x.500 and password) to obtain your latest immunization information. OHS staff must have this information to provide immunization services. |
