Immunization Compliance Requirements


Introduction: This page provides access to information that outlines how to be compliant with immunization requirements when you start in your program and throughout your training.

All residents and fellows must meet University of Minnesota Academic Health Center requirements for immunization compliance throughout their residency/fellowship.  Your immunization record is maintained by Boynton Health Service.

Incoming Residents/Fellows:
Incoming residents and fellows must have and document the immunizations set forth in Dr. Barbara Brandt's memo prior to their program start date.  You have the option of declining the Hep B vaccine.  If you decline, you must complete the Hepatitis B Vaccine Declination form and submit it directly to Boynton Health Services.  If you have not completed all immunization requirements prior to your start date, you must attain compliance as soon as possible with your primary care physician or clinic.  The University does not cover these costs.  Boynton Health Services will charge you for these services.

Complete this form if you are declining the Hepatitis B vaccination. Click here to access your University Portal. Use this form to authorize your health care provider to release your immunization history to the Boynton Health Services Occupational Health

**The BHS Clinic held each month in the CHIPS lounge is for medical students only. Residents/Fellows cannot use this clinic.  They must use one our EHS offices.  See information below under Annual Mantoux.

Continuing Residents/Fellows:
Continuing residents/fellows are required to get their annual Mantoux in August of each year.  No appointment is necessary and there is no charge for this service if you go to one of our approved Employee Health Services Offices.  Boynton Health Service does not participate in the annual Mantoux campaign and they will charge you for this service.  See Annual Mantoux information below for details.
