Benefits: Dental Insurance


Coverage
Single dental coverage only is offered through Delta Dental Plan of Minnesota.  The plan offered is Delta Dental PPO Option which pays 100% for most preventive and diagnostic services.  Other services are subject to applicable co-pay. Delta Dental is an employer-sponsored plan.

Enrollment
Enrollment is optional at your date of hire.  If you do not choose to enroll at that time you will not be able to enroll until the next year of open enrollment which is every two years.

Billing
Details about your dental insurance biweekly payroll deduction are available through your department payroll office.  All dental insurance premiums are handled through the University of Minnesota Payroll Department and monitored by your department payroll office. 

Questions on Coverage
Contact Delta Dental PPO Option customer service at 651-406-5916 or 1-800-553-9536.    Customer service will request your Subscriber Number which is your Social Security Number.  Information about Delta Dental of Minnesota is available on their web site:  www.deltadentalmn.org .

Extension of Coverage
A trainee has the option to continue dental insurance coverage for a period of up to eighteen (18) months after leaving a program through COBRA, unless other conditions apply.  You will be required to pay your own dental insurance premium plus a statutory administrative fee of 2% for the months you choose to extend your insurance.
