Policy
All trainees who successfully complete all requirements of their training program must receive a certificate of completion.  Programs must retain a scanned or hardcopy of the certificates on file in the event that the original is lost or damaged.

Certificates must not be reprinted for trainees requesting to have their name changed as the certificate must reflect their legal name at the time of graduation. 

Programs must not change the medical degree that was originally bestowed (i.e. MBBS to MD).

If a certificate must be recreated due to loss or damage, the program must obtain signatures from all of the original signers at the time graduation. If you are unable to obtain the signature of an original signer, contact GME Administration.
