Registration and Medical Insurance
If you are a new patient, or if your insurance or home address or phone has changed, please contact the Outpatient Registration Department at:

Please bring your insurance card with you to your appointment. Insurance co-payments or deposits for self-pay patients are due at the time of service. The clinic accepts major credit cards, checks and cash payments.

If required by your health plan, please obtain a referral in advance of your visit. Referrals cannot be obtained after the visit has occurred and insurance may deny coverage or pay at a lower benefit level, leaving the patient responsible for a larger portion of the bill.
