Make a Gift


Thank you for your interest in supporting the work of the Division of Adult Hematology, Oncology and Transplantation at the University of Minnesota.

There are four ways to make your gift in just minutes:

  • Send check
    made payable to the Minnesota Medical Foundation
    directly to Cheri Ptacek (Director of Operations) at Division of Adult Hematology, Oncology and Transplantation; Mayo Mail Code 480; 420 Delaware Street SE; Minneapolis, MN 55455 with a note stating what the check is for, which doctor should use the money, if any, and name of person the memorial is in. If you would like your gift acknowledged to a family member, please provide that name and address. Should you have any questions or concerns, please contact Mrs. Ptacek at 612-625-2654.
  • On-line through a secure, on-line gift form (see link below)
  • By check made payable to the Minnesota Medical Foundation, PO Box 64001, St. Paul, MN  55164-0001, also with a note stating what the check is for, which doctor should use the money, if any, and name of person the memorial is in.
  • By phone with your credit card, by calling 1-800-922-1663 or 612-625-1440.

Gifts to the Division of Adult Hematology, Oncology and Transplantation are received by the Minnesota Medical Foundation -- a University of Minnesota-related nonprofit organization.   Whether you make your gift on-line, or by mail or phone, please be sure to specify the Division of Adult Hematology, Oncology and Transplantation.  You may also direct your gift to a specific area of research or education within the Division of Adult Hematology, Oncology and Transplantation.  Simply indicate the area you wish to support.
