University of Minnesota
Program of Mortuary Science
Scholarship Application Instruction Form
Application Deadline: March 25

This form explains the process of applying for a scholarship award from the following Program of Mortuary Science Scholarship Funds:

• The James Ellison Memorial Scholarship Fund
• The Minnesota Funeral Directors Association Past Presidents’ Scholarship Fund
• The Robert C. Slater Scholarship Fund

Applications will be reviewed around April 1st of each year. Students who are awarded scholarships will be notified of the amount of the scholarship around April 15th. The amount of the award will be based on several factors, including (but not limited to) financial need, amount of funds available for distribution, and the number of applications for scholarships received. The minimum amount of scholarship money to be awarded to each recipient will be $1,000.

To be eligible to apply for a scholarship, the student must be matriculated (formally admitted to, and enrolled in) the Program of Mortuary Science at the University of Minnesota. The student must have completed at least 13 semester credits as a mortuary science major, have a Program grade point average of at least 2.5, and be enrolled as a full-time student. Scholarship winners will have their award credited to their student account, at the start of an academic term of their choosing. The money must be used to offset the cost of tuition.

To apply for a scholarship, the student must submit the following materials to the Scholarship Award Committee:

1. A brief outline of the student’s work and volunteer / service activity history.

2. Copies of all the student’s college transcripts (A copy of a student’s APAS report, downloaded from the U of M “One Stop” website, is acceptable.)

3. Documentation of financial need. A copy of the student’s FAFSA form will be sufficient. If a student did not complete a FAFSA form, other documentation, such as a copy of the student’s tax return for the most recent year, should be submitted. Any additional supporting document(s) the student may wish to include, for the purpose of evidencing their financial need to the Scholarship Award Committee, may also be submitted.

4. A brief Personal Statement of approximately two double-spaced, typewritten pages, in which the student discusses an issue relevant to funeral service today, and how the student thinks they will work to address this issue once they graduate from the Program.

Once the student has compiled all of these materials, the documents should be placed in an envelope and submitted to

Program Director Michael LuBrant
MMC 740
420 Delaware Street SE
Minneapolis, MN, 55455

As application materials are received by the Program, copies of the documents will be generated, with personally-identifiable information blacked out. The personally-unidentifiable documents will then be forwarded to Scholarship Award Committee members for “blind” review. If you have any questions about the application process, please feel free to contact Michael LuBrant at mpl@umn.edu.
