Coordination of Care Between Clinic Visits

Patient Care Coordination Between Clinic Visits

Our patient care nurse coordinators are available to assist patients with sick calls, pharmacy calls, coordination of home health services, documentation required for TEFRA, Social Security, disability claims, etc.  Please keep in mind that our volume of calls is high and each call is returned as soon as possible.  Priority is given to returning the most urgent sick patient calls.  Messages left after regular office hours are generally not returned until the next business day.

Monday through Friday, 8:00 am – 4:30 pm
Contact a Patient Care Coordinator at 612-625-5995 or 1-800-688-5252, ext. 55995

Renew Prescriptions

Between Clinic Visits

  1. Call your pharmacy yourself to re-order your supply.
  2. If there are no further refills on that prescription, the pharmacy will call our CF Center office to get refill orders.  Please allow 1-2 days for us to respond to that request.

At a Clinic Visit

  1. Tell the nurse/physician you need renewals.
  2. When the physician orders a new medication, have him give you the new prescription.
  3. Be sure you understand why the medication is being ordered. Ask if you have questions about its purpose and length of time to be used, etc.
