What To Expect At Your Appointment

Dear Parent:
Thank you again for choosing the Autism Spectrum Disorders Clinic at the University of Minnesota Medical Center for your child's medical care. Please read this letter carefully to insure that your appointment goes smoothly. We would like your appointment to be as helpful for your family as possible.
WHAT TO EXPECT
The Autism Spectrum Disorders Clinic was created to provide a service to patients and families with autism and related disorders. The clinic provides several services, which include diagnosis, recommendations for treatment, follow up support and research.
To facilitate the visit, we need to acquire certain information prior to scheduling your child. The Patient Intake Questionnaire should be completed and returned to the clinic. We realize that some of the questions may not be pertinent to your child's situation or may be difficult to answer for other reasons. Please fill out the questions to the best of your ability. In addition, in order to provide a comprehensive evaluation for your child, there are several components that are needed.
First, depending on the age of your child, cognitive (IQ)/developmental, language and adaptive behavior (life skills) assessments must be completed prior to your child's autism-specific evaluation. If the testing has been completed within the past 3 years at your child's school or at another clinic please send it to us with the completed Autism Spectrum Disorders Questionnaire. If your child has not had any or a portion of this testing within the past year and if you would like it to be completed by us, we can administer this testing at our clinic at the University of Minnesota. Once we have received your completed questionnaire and any previous reports including cognitive/developmental language and adaptive behavior evaluations, one of our staff members will contact you to schedule the appropriate appointments.
A developmental behavioral pediatrician initially sees each patient. The developmental behavioral pediatrician will provide recommendations for any needed medical testing or treatment.
Your child's medical records are also required in advance to facilitate the evaluation process. Please include the following items to the clinician if your child has these completed:
- Any X-rays, prior laboratory tests, EEG reports, medical records, psychological and development evaluations, school reports, IEP testing and individual education plan (IEP).
- A list of the medications your child is taking.
These records can be mailed or faxed to the Autism Spectrum Disorder Clinic at the address/number below by the institution or medical facility.
