Making an Appointment
To schedule an appointment, please call (612) 625-5411. To help us provide you and your family with the best care possible, please send your child's medical records before your child's first appointment. For new patients, our scheduler will contact you to ensure your child's records are in order prior to the first visit.
Please confirm with your referring physician that we receive the following:
- All laboratory reports
- X-rays
- Pathology slides
- Most recent physician notes

Your Child's First Visit
Your child's first visit is typically an exam and consultation with a pediatric hematologist oncologist. The amount of time needed per patient varies, therefore, please reserve up to three hours for the first appointment. Following the visit, your child's case will be discussed by the multidisciplinary team to determine whether additional tests are needed or recommend the course of treatment. Your child will then have a follow-up visit with the hematologist oncologist to discuss your treatment plan and options. Because your child's personal support "team" plays and important role in the fight against cancer or any illness, we encourage family members to attend any or all appointments.
Registration and Medical Insurance
Please bring your insurance card to the appointment. Insurance co-payments or deposits for self-pay patients are due at the time of service. The clinic accepts major credit cards, checks and/or cash payments. If your health plan requires a referral for services obtained in this clinic, please obtain a referral from the appropriate provider in advance of your child's visit.
Referrals can not be obtained after the visit has occurred and insurance may deny coverage or pay at a lower benefit level, leaving you responsible for a larger portion of the bill.
