Medical Licensure


Here is the information you need to take Step 3, get licensed, and obtain a DEA number.  You’ll need step 3 to get a license, and a license to get a DEA.

For your License application, you will need two peers with a license to sign for you…any chief resident, the associate program director, or program director would be happy to do this for you.

Anytime you need something notarized, Juliet Massie in the Pediatric Education Office (PWB 13-168) will be happy to do so free of charge.  Please send an e-mail to juliet@umn.edu before you come to ensure she will be in the office.

You can use your educational allowance to pay for any of these registration fees.

NOTE:  Make sure your printer is functioning properly before registering, as you may be charged TWICE if you have to or are forced to re-print a page

OR:

Don't forget to download the generic Post Graduate Training Form from the Step 3 Instructions page.  This is a form that verifies that you are a pediatric resident.

To find a test center or schedule an exam:

  • Go to http://www.prometric.com/Candidates/default.htm
  • Your Area of Study is “Professional Licensure & Certification”
  • Your Testing Program is “United States Medical Licensing Exam: Step 1, Step 2, and Step 3”
  • Click on Locate a Testing Center or Schedule, Reschedule, Cancel, or Confirm an Exam

Minnesota licensure (passing of Step 3 required for licensure):

NOTE:  Licensure is done only every few months so pay close attention to deadlines!

OR:

After you submit your packet, you will be notified of a day to come for a “meeting” to obtain your medical license.  It takes an hour or two and involves watching a video and very briefly meeting with someone from the board for them to sign your papers.

  1. Under Business Category select "Form 224"
  2. Under Business Activity choose "PRACTITIONER"
  3. Click "Begin"
  4. Use the Pediatric Department’s address as the business address:

  5. Sign up for schedule 2 - schedule 5 (not schedule 1)
  6. Under “4a” there is no State Controlled Substance No. for Minnesota
  7. Print out, sign, and mail in application
