Directions for Completing the Applications


Applicants are considered completed when all of the following materials have been received by the University of Minnesota. Only complete applications will be reviewed.

1. A completed and signed APPIC Uniform Application form. This form is available from the APPIC web site: http://www.appic.org/match/5_3_match_application.html.
2. A completed and signed supplemental University of Minnesota Medical School Psychology Internship application form.
3. A curriculum vitae.
4. Two psychological assessment reports written by the applicant.
5. One psychotherapy treatment summary written by the applicant.
6. Official current academic transcript sent directly from the applicant's graduate training institution(s).
7. Official current academic transcript sent directly from the applicant s undergraduate training institution(s).
8. The Director of Training of the applicant's graduate psychology training program must complete and forward the "APPIC Application for Psychology Internship (AAPI) PART 2: Academic Program's Verification of Internship Eligibility and Readiness" form. This form is available from the APPIC website: http://www.appic.org/match/5_3_match_application.html.
9. A minimum of two other letters of recommendation from faculty or clinical supervisors who are familiar with the applicant s clinical work.
10. Applicants also must be registered with the National Matching Service.
