Welcome to the Diabetes Education and Self Management Program at Fairview-University Medical Center. Our program has been in existence since 1997 and became recognized by the American Diabetes Association in April, 1998.

Program Services:

• Comprehensive self-management training for children and adults.
• Personalized instruction by experienced, registered nurses and dietitians who are Certified Diabetes Educators
• Ongoing follow-up and support after your education is complete.

Our program is specially designed for patients who are:

• New to diabetes.
• Children or adolescents with diabetes.
• Beginning insulin or diabetes medication.
• Desiring to lose weight.
• Pregnant or planning a pregnancy.
• Requiring adjustment of insulin, oral medication, or diet and exercise plans.
• Seeking better blood sugar control through intensive medication management, carbohydrate counting, or insulin pumps.
• In need of a diabetes update or have not seen a diabetes educator in the past 5 years.

Scheduling an Appointment:

You need a referral from your physician to participate in the program. You will also need to register with Fairview-University Medical Center. Please have your insurance information available for this process.
