Administrative Requirements, Division of Pediatric Endocrinology at the University of Minnesota

Prior to beginning the fellowship, the following need to be addressed:

  1. The Residency/Fellowship Agreement, which will be mailed to the fellow about one month prior to the fellowship, must be filled out and returned (contact: Deb Slavin 612-626-6910).
  2. The background study form, which will be mailed to the fellow about one month prior to the fellowship, must be filled out and returned (contact: Deb Slavin 612-626-6910).
  3. The Immunization Compliance Form, which will be mailed to the fellow about one month prior to the fellowship, must be filled out and returned. Immunizations not up to date must be completed prior to starting the fellowship or may be completed immediately upon starting. For example, a two-step mantoux is required¿this will be done free of charge at Boynton Health Services if the fellow has not already had this (contact: Deb Slavin 612-626-6910).
  4. A residency permit or a physician license for the State of Minnesota should be sought approximately one month prior to fellowship. Doreen in the Pediatric Endocrinology office can help with this (612-624-5409). Only the residency permit is required for fellowship, but moonlighting will require a license.
  5. Verification of immigration and VISA status as well as a copy of an ECFMG, if applicable. Certificate indicating the validation dates, if applicable, for all International Medical Graduates. Our NIH training grant requires U.S. citizenship or permanent residence.
  6. For those fellows pursuing a Master's Degree in Clinical Research, you must apply on-line to the graduate school in April. The coordinator of the program is Shelley Cooksey (612-626-8803). You will get a student ID number at this time. Once you have an ID, you can get University e-mail . You will receive instruction on this at the time you apply, but if you have any questions you can call the e-mail helpline at 612-626-4276.

Immediately upon starting fellowship, the following need to be addressed:

  1. HIPAA training needs to be completed on line within the first 90 days of fellowship.
  2. Registration for the Master's Program occurs in August.
  3. Parking, obtaining an ID from security (612-273-4533), ordering white coats and signing up for hospital computer access can all be done in the first week with help from Doreen (612-624-5409).
  4. Fostering Integrity in Research, Scholarship and Teaching (FIRST), formerly known as Responsibility in the Conduct of Research (RCR). There are two half-day courses that need to be completed during the first year of research. Each is offered twice a semester.
